Health Connections Mendip is a small team working within the NHS supporting people living in Mendip to access the help they choose to manage their health and wellbeing. We are looking for Health Connectors to be part of this innovative and dynamic new service. The role we are currently recruiting for is:
2 x Health Connectors
Employed by Frome Medical Practice one post will be based primarily in Frome and one in Shepton Mallet. You will need to be able to travel across Mendip when required. Shepton post is 37.5 hours a week. Frome based post is 22.5 hours a week to include one day working across Mendip leading group education sessions. The posts are fixed term until March 2017.
